To make your first visit as smooth and efficient as possible, we recommend completing your new patient forms ahead of time.
These forms give us important background about your medical and dental history, allowing us to better understand your needs and provide care that is tailored to you right from the start.
Transferring Your Dental Records
If you’re joining us from another dental office, you can also complete a records release form. This allows us to request your previous dental information directly, so we have a complete picture of your oral health history without you needing to do the extra work.

If you prefer to complete them on-site, we recommend arriving a little early so we can keep your appointment running smoothly.
